Medical and Vocational Case Management Services

We use the biopsychosocial approach to case management addressing physical, psychological and environmental aspects of every case from a medical and vocational rehabilitation perspective.

Our medical case management aims to achieve as complete a recovery as possible and will usually involve some or all of the following:

  • Liaison with the NHS to achieve optimal recovery as early as possible
  • Co-ordination of private provision where appropriate to achieve recovery when NHS provision cannot achieve the party’s goals
  • Liaison with and management of specialist consultants and diagnostic services to identify pathways to rehabilitation
  • Provision of specialist aids, adaptations and transport/living support to aid and support recovery
  • Organising medical care and therapies
  • Establishing and monitoring of care regimes

Vocational case management helps the injured person return to work in their original capacity or in a modified/alternative role with their current employer. If this is not possible, we assist the injured person into a new role with a fresh employer or to be able to engage in some level of economic activity. This will involve a number of strategies including;

  • Liaison with employers to facilitate graduated return to work programmes
  • Identifying alternative job roles with the employer
  • Workplace ergonomic assessments and reasonable adaptations to allow the injured person to continue working
  • Liaison with state job placement and support services to identify opportunities for alternative employment
  • Conduct job and transferable skills analysis to establish job and career options
  •  Assess the suitability of alternative work options in conjunction with treating medical consultants 

For Each and Every Case, we Provide:

  • Initial assessment
    A suitably qualified rehabilitation case manager will conduct an initial assessment by telephone or face to face (as required by the needs of the case) and report on the outcome
  • Dedicated case managers
    All cases are managed from beginning to end by the same case manager managing a multi-disciplinary team
  • Rehabilitation plans
    We will provide a goal orientated rehabilitation action plan with anticipated outcomes, costs and timescales agreed by all stakeholders
  • Ongoing communication
    Our dedicated case manager ensures that all parties are kept up to date with progress throughout the life of the case
